Joint Commission International's Clinical Care Program Certification (CCPC): Fostering Better Outcomes through Integration and Coordination of Care




STANDARDS

Beginning January 2010, Joint Commission International Disease or Condition-Specific Care Certification will have a new name: Clinical Care Program Certification (CCPC).

JCI began offering DCSC Certifications in 2005. Since that time the program has grown beyond the management of a program for diseases and/or conditions. This new name reflects the expansion of the program to include 15 categories of clinical services that are now eligible for certification.
                                                                                                                                                                                    
JCI's CCPC standards evaluate and certify excellence in clinical programs that are part of an organization's overall patient care services, including:

· Heart Failure
· Acute Myocardial Infaction
· Primary Stroke
· Diabetes Mellitus (Type 1) and/or Diabetes Mellitus (Type 2)
· Chronic Kidney Disease (Stage I to IV)
· End Stage Renal Disease
· Palliative Care (any type of palliative care)
· Traumatic Brain Injury
· HIV/AIDS Management
· Cancer (any type of cancer)
· Pain Management
· Asthma
· Joint Replacement (any type of joint replacement)
· Transplantation (any type of transplantation)
· Chronic Obstructive Pulmonary Disease (COPD)

 

Certification differs from accreditation in that accreditation focuses on an organization's overall commitment to quality through organization-wide, patient-centered and health care organization management systems and processes. Certification demonstrates excellence in fostering better outcomes by the integration and coordination of care for treatment of a specific disease (such as Diabetes Mellitus Type 2), condition (such as Acute Myocardial Infarction), or clinical care service (such as Joint Replacement). CCPC is based on a clear assessment of consensus-based standards and criteria, the effective use of clinical guidelines, and an organized approach to performance measurement and improvement.

JCI program certification reflects a visible commitment by the host organization's leadership and staff to excellence in patient safety and quality care. Achieving certification assures patients, insurers, and other organizations that pay for care of the program's capacity to provide care based on the best international practices.
